2 Injured in Semi-Truck Crash on I-5 Near I-405 Interchange in Portland, OR
Portland, OR (April 25, 2026) — Two people were seriously injured Saturday morning in a crash involving two semi-trucks on northbound Interstate 5 near the I-405 interchange, just south of the Marquam Bridge. According to the Portland Police, the crash was reported around 10:40 a.m.
Investigators said one semi-truck driver allegedly did not slow for traffic ahead, swerved, and struck another semi-trailer. The driver of that truck and a passenger were both seriously injured and transported to the hospital. The other truck driver was not injured. Authorities have stated that no citations have been issued and the investigation remains ongoing.
Looking Beyond the Initial Explanation
Early reports suggest the crash involved a failure to slow for traffic, but that does not always explain why it happened. In situations like this, investigators often take a closer look at whether the truck responded as expected when braking was needed.
A sudden need to slow down can reveal problems that are not obvious during normal driving. That is why it is important to look at more than just the driver’s reaction in the moment.
Could Mechanical Issues Have Played a Role?
When a semi-truck is unable to slow in time, one of the first areas investigators may examine is the braking system. Commercial trucks rely on complex air brake systems, and any issue with pressure, response time, or maintenance can affect how quickly the vehicle slows down.
Investigators may also review tire condition, load weight, and how the truck was handling leading up to the crash. If a truck swerved after failing to slow, that can raise additional questions about steering or stability.
Reviewing Maintenance History and Equipment Condition
Another key step is reviewing the truck’s inspection and maintenance records. Commercial vehicles are required to meet strict service standards, and those records can show whether the truck had any known issues before the crash.
If the truck had prior brake problems, worn components, or missed inspections, that could become an important part of the analysis. On the other hand, if the equipment appears to have failed unexpectedly, investigators may need to consider whether a component did not perform as intended.
Why the Current Lack of Charges Matters
The fact that no citations have been issued at this stage suggests that investigators are still working through multiple possibilities. Without clear findings, it is important not to assume a single cause.
A full investigation will help determine whether the crash was the result of driver action, equipment condition, or a combination of both.

Why Experience in Commercial Truck Crashes Is Crucial for Accident Claims
When a crash may involve braking issues, equipment failure, or maintenance concerns, the focus often shifts to how the truck was maintained and whether it was safe to operate. These cases can require a detailed review of both driver actions and the condition of the vehicle.
Since the operation of a truck is highly regulated, it is important to consider hiring an attorney experienced in truck crashes specifically if you find yourself in a similar situation. Attorneys certified by the National Board of Trial Advocacy in truck accident law have demonstrated experience handling these types of cases, and few attorneys in the Pacific Northwest hold that certification. Tom D’Amore, founder of D’Amore Law Group, has more than 30 years of experience and is certified by the National Board of Trial Advocacy.
